Volunteer Application For Adult Workers In Children’s / Youth Ministry

Thank you so much for your interest in working with our students here at lifepointe church. This application is to be completed by all volunteer applicants who are involved in the supervision or custody of minors. This is not an employment application form. The purpose is to help the church provide a safe and secure environment for those children who participate in our programs and use of our facilities. (All information is confidential)
